Job Description:
Start Date: ASAP (up to 4 weeks credentialing) - 3 months
Desired Schedule: Monday - Friday from 8:00am-5:00pm (no call)
Scope of Work: Average of 5 patients per day; manage chronic
disease states of geriatric population; must work with care plans
within group of: NP, IM Doc, PT, OT, Dietician, and other
specialties for each patient; manage dialysis treatment set up;
coordinate all care for these patients daily EMR: EMR is E-Clinical
works (can train) Requirements: Board Cert in IM/FM, CA license,
DEA, BLS, Geriatric experience, PACE program or Team based practice
experience, 2 references from DO/MD
