Nurse to Patient Ratio: Acute care case management is very fast paced and the case manager assignment can vary from 15-20+ patients.
Charting: Cerner, Allscripts
Scrub Color: scrubs not required

The hospital case manager role is assess, develop and coordinate a safe discharge plan with the patient, family, hospital team to home, with home health, to an alternate level of care, skilled nursing, LTAC and acute to acute transfers.
The case manager is required to follow all the Medicare regulations, providing Observation notices, IMFMs, Patient Choice, understand the process when a Medicare patient appeals there discharge.
The CM uses the electronic health record along with our documentation system, Allscripts, which we also use to document interventions, avoidable days, send and track referrals for home health, skilled nursing facilities.Required QualificationsBLS2 years acute care experiencePreferred QualificationsCernerFacility LocationThe San Francisco Bay Area is a favorite destination for anyone who thrives on fun, excitement and endless variety.
