Job Description:
Primary Purpose The Clinical Case Manager is responsible for
conducting pre-screenings of students seeking mental health
services, serving as a liaison between Health & Wellness Services
and community partners, and assisting students who require medical
and mental health services off-campus. Acts as a student health
advocate and liaison between internal and external health care
offices and facilities to ensure that students reach and maintain
their optimum level of physical and mental health. Works
collaboratively as part of a multi-disciplinary team in meeting the
physical and behavioral health needs of our student population.
Essential Functions Perform pre-screening of students requesting
counseling services and refer students to appropriate level of
therapeutic care. Consult with and make referrals to campus
partners that can help support students’ retention and academic
outcomes. Utilize knowledge and assessment of mental health needs
to assist with or facilitate referrals to off campus community
resources when the student needs are outside of the scope of
practice for SHS or CAPS . Services may include but are not limited
to direct psychiatric admissions, outpatient and long-term mental
health services, health services, legal services, or social
services. Assist students requiring specialist care to schedule
appointments, complete paperwork, and obtain transportation to
increase compliance with recommended treatment plan. Serve as a
point of contact for faculty, staff, parents, and students to
express concerns regarding student behavior for referral to
appropriate campus and/or off campus resources. Completes outreach
to students identified as needing mental or physical health support
to help ensure these students connect to appropriate campus and
community resources. Primary outreach contact to students who have
been hospitalized to ensure the coordination of care and referral
to appropriate campus partners. Provides guidance and consultation
to the Dean of Students, Care Managers, Accommodation Specialists,
and other critical campus partners, related to student support,
outreach, and intervention strategies. Participation in Dean of
Student office meetings as appropriate, including Care Team,
Student of Concern, and the Behavioral Intervention Team. Work with
student clients nearing therapy session limits to identify
specialized referral services on and off campus. Collaborate with
the Insurance Coordinator to communicate with various insurance
plans to assist with connecting students to off campus resources.
Assist students with release of information documents and consent
forms along with retrieving outside records when necessary, for
continuum of care. Establish working relationships with community
providers to expedite referrals. Engage in ongoing communication
and follow up with students and external medical, mental health,
and substance abuse providers to assure continuity of care/service
and completion of scheduled appointment. Maintain up to date mental
and physical health referral guides for students on all 3 campuses.
Consult with counselors and Clinical Services providers as
appropriate. Facilitate presentations, training, and outreach
programming for campus community related to mental health and
services provided CAPS and SHS . Maintain an in-depth knowledge of
Health and Wellness Services and university policies, procedures,
regulations, and functions of individuals departments. Serve on
university-wide committees as appropriate. Perform other related
duties as assigned. University of the Pacific recognizes that

students, faculty, and staff. Minimum Qualifications Master’s
Degree in Psychology, Social Work, or related field Three (3) years
of experience working in student support services in Higher
education. One (1) year experience utilizing a referral/case
management system; knowledge of Student Information Systems and
Learning Management Systems. Preferred Qualifications Five (5)
years related professional experience in working with a diverse
student population. Demonstrated experience working with diverse
communities including but not limited to college and young adult
age populations, individuals of various racial/ethnic, religious
backgrounds. Experience working with students with complex needs
and/or limited participation. Effective communication skills and
demonstrated ability to express ideas clearly and concisely both
verbally and in writing, especially the ability to simplify
detailed information and articulate complex ideas in a manner that
is easily understood. Knowledge of federal and state laws
concerning confidentiality of student records. Must possess
critical thinking, problem solving and conflict resolution skills,
and the capacity to work and make decisions autonomously.
Interpersonal skills to engage with students and staff members to
effectively achieve desired goals. Organizational, time management
and computer skills. Excellent written and verbal communication
skills. Experience and sensitivity in working with people of
diverse backgrounds and cultures. Demonstrated experience in
advancing social justice, equity, and inclusion in a university
setting. Ability to engage and integrate culturally responsive
practices and knowledge in their work. Work Schedule Work is
generally performed during standard business hours. May be required
to work non-business hours.
